No Show/Cancellation Policy and Facility Guidelines:

Evolve Medical Associates value our patients and their time.  Our staff takes every initiative to schedule all appointments fittingly.  EMA asks our patients to please adhere to the following policies/guidelines to provide you with the best possible experience.

  •  For enhanced comfort and relaxation, please arrive at least 15 minutes prior to your scheduled appointment.  To ensure the correct procedure has been scheduled, please be prepared to answer some general questions regarding your skin care concerns/goals and medical conditions (if applicable). 
  • Late arrivals will limit your service time, thus lessening effectiveness and/or pleasure.  Your treatment will end at the scheduled time to accommodate the next patients scheduled appointment.  The full value of your treatment will apply.
  • A deposit of $50 will be required prior to making an appointment after the second time of failing to notify us of a schedule change within 24 hours of your appointment.  (This would require a credit card to be placed on file.) If a third “No Show” occurrence happens within six months, your card will be charged for the full value of your scheduled treatment.
  •  In order to establish optimal relations with our patients and avoid misunderstanding regarding our payment policies, our staff is trained to inform you of the financial policies of this office. PAYMENT IS EXPECTED OF YOU AT THE TIME OF SERVICE.  We gladly accept the following payment options: VISA, MASTERCARD, AMEX, DISCOVER, CARE CREDIT AND CASH.  WE DO NOT TAKE CHECKS OF ANY KIND
  •  On your visit, personal belongings are the full responsibility of the client.
  •  The ensure the safety of children and the enjoyment of all clients, we ask that parents or guardians make other arrangements for children while they are receiving services.
  •  Please place cell phones/pagers to vibrate/silence while in our facility.

 

Refund Policy for Services/Packages

Effective May 1st, 2009

Evolve Medical Associates will gladly refund your money for unused services/packages 30 days after the purchase date (This includes sales by phone, online, in person or outside event). 

 The following guidelines apply to refund of services/packages or partially used services/packages:

  1. Client must have a copy of the original receipt that includes the date of purchase, method of payment(Credit Card purchases must go back on the original card) and service provider**.   
  2. We are not able to refund partially used services such as injectables, Laser Hair Removal, Peels, Spray Tanning, etc***.
  3. Services are refunded at the original purchase price unless the service is listed at a new lower price, the service will then be refunded at the new price point.
  4. Open House, Holiday, and special event packages/services are specially priced and expire six (6) months after the purchase date.
  5. Certain series packages may require a period of 12 months to complete, these packages will expire one year after purchase and are non-refundable.
  6. Special coupons or certificates provided by Evolve Medical Associates or affiliates, have no cash value and may not be refunded, transferred or extended.

 Evolve Medical Associates has the right to deny any refund, transfer or exchange of services. In some cases we may exchange services for gift certificates or gift cards that will have a default expiration date of 6 months unless indicated otherwise.  Medical services such as sclerotherapy, that are billed to insurance or purchased by client without insurance are non-refundable and non-transferable to other clients nor for other services thereof.

*We may require additional information such as your current mailing address  and a written request for the refund.

**We may offer you the opportunity to exchange services for services of the same category. ex: You are not able to use a Microdermabrasion, so we will exchange that service for another Esthetician service like a peel or facial that is of equal or lesser value.

NOTE: Granted Refunds may take up to 10 business days to process.
