Simply pay your minimum monthly payment and pay off the entire balance by the end of your promotional period and you pay No Interest. WHAT IS A CARECREDIT PAYMENT PLAN?
CareCredit offers a full range of payment plans. Our 3, 6, 12, & 18-month No Interest* payment plans feature monthly payments as low as 3% of the balance and let you pay within the specified time without incurring interest charges. Simply make your minimum monthly payment and pay off your entire balance during the agreed-upon time period, and you’ll pay no interest.
For patients/clients with higher treatment fees who would prefer more time to pay with lower monthly payments, our 24, 36, 48, & 60-month extended payment plans* offer a low, fixed 11.9% interest rate and an extended term.

HOW CAN I APPLY FOR CARECREDIT?
You can apply online or download and print the application and bring it to Evolve. The application is quick and easy, and after it’s submitted, you’ll instantly learn if you’re approved.
WHAT INFORMATION DO I NEED TO APPLY ONLINE?
To begin your online application, all you have to do is choose from one of the three options: search for your doctor by name, choose from a list of doctors in your area OR simply tell us how you’re planning to use CareCredit, if you’re not ready to choose your doctor yet.
We will obtain and verify information that identifies you as a new account holder. We’ll use your name, address, date of birth, Social Security, Driver’s License, and other information for this purpose.

PROMOTIONAL PLANS
You will get the promotional interest rate (either No Interest or low interest 11.9%) if you pay the minimum payment on the account when due and pay the account in full within the pre-determined promotional period. Not all promotions are offered by all CareCredit providers. Please see your doctor to find out which plans are available.
NON-PROMOTIONAL INTEREST RATE
This rate is a non-promotional interest rate (APR). This interest rate only applies if you do not meet the terms of the promotion. You would only pay this interest rate if you do not pay off your account in full during the pre-determined promotional period or you do not make the minimum payment on the account when due. DELINQUENCY RATE
If you do not make your required Minimum Payment by the Payment Due Date, you make a payment that is not honored by your bank or you exceed your credit limit, the Delinquency Rate, or at our discretion a lower rate (rather than the Standard Rate), may apply to all existing balances on your account and all new transactions beginning with the first day of the billing period in which your missed or returned payment was due or in which you exceeded your credit limit.

Approval is based on information from your credit application and past credit history. This includes information from the primary applicant as well as any co-applicant.

WHEN WILL I RECEIVE MY FIRST CARECREDIT BILLING STATEMENT?
You won’t receive a billing statement from CareCredit until a charge has been processed on your account. Once a charge has been processed, you will receive your first billing statement within 30 days.
CAN I PAY OFF MY CARECREDIT ACCOUNT BEFORE THE END OF THE TERM?
Yes. There are no pre-payment penalties with CareCredit.
I ALREADY HAVE A CARECREDIT ACCOUNT, CAN I USE IT AGAIN?
Yes, you can use your CareCredit account over and over, at any practice which accepts CareCredit. As long as you have the available credit on your account, you can use your card for ongoing or new treatments and procedures, without reapplying.

CAN I PAY A PORTION OF THE TREATMENT COST MYSELF AND USE CARECREDIT FOR THE REMAINDER?
Yes. You can use CareCredit for treatment balances from $1 to over $25,000.
